Risks include, but are not limited to, scratches, bruises, sprains, broken bones, concussions, back and joint injuries, heart attacks, animal bites, kicks, or head butts. I, the undersigned, my heirs, and assigns assume all risks of loss, damage, or injury, including paralysis or death, that may occur or be sustained while employed at any time, whether during regular duties, special projects, or during any event or activity sponsored by De Vor Dairy Farm And Creamery. I know, understand, and appreciate these and other risks that are inherent in working at De Vor Dairy Farm And Creamery. I hereby assert that my employment and participation is voluntary and that I knowingly assume all such risks throughout the calendar year from which this form is signed.
